What to expect on your visit
You may print out the registration form below and fill it out before your visit. This will help shorten your wait time so you can get treated and feeling better sooner!
Upon completion of registration, a staff member will take you into our private exam room where your evaluation will begin. The provider and patient together will then formulate a plan of diagnostic testing as needed. Testing may include X-rays, blood and urine tests or an electrocardiogram (EKG). This information will be used to assist in making a diagnosis and treatment plan. It is always recommended to follow up with your primary care doctor after an urgent care visit.

During checkout, you will receive all necessary prescriptions and instructions given by your provider. The visit will also be documented in your personal medical file.

MyHealth Urgent Care makes all reasonable efforts to comply with the HIPAA (Health Insurance Portability and Accountability Act of 1996) and privacy regulations. Please read our privacy practices and be prepared to sign a Patient Authorization Form when you visit.
Care for Minors
Please note: Minor patients under the age of 18 must be accompanied by a parent or legal guardian. Any other accompanying adult must have a notarized statement signed by the parent or guardian authorizing treatment.
We are here to help take care of the many nonlife-threatening emergencies that are common among adults and children of all ages (including infants over six months). We do our best to keep the paperwork to a minimum so that you can get on with getting better.
