9. As a patient of Meadowbrook Urgent Care, what are my rights and responsibilites?
Patient Rights and Responsibilities
At Meadowbrook Urgent Care, we strive to provide the best patient care possible. Unlike ER's, we take the time to sit and talk to you, answering every single question you may have. We want you to have the trust in us to be there for you when unexpected and unfortunate health problems arise. As a patient of Meadowbrook Urgent Care, you have both rights and responsibilities that will assist us in providing you with quality health care.
Patient Rights
· No discrimination. No patient will be denied appropriate care on the basis of race, religion, color, national origin, sex, age, disability, marital status or sexual preference.
· Appropriate care. Every patient is entitled to receive adequate and appropriate care.
· Informed decisions. Every patient has the right to make informed decisions and to receive information about his or her medical condition, proposed course of treatment, prospects for recovery and available choices for treatment.
· Inspection of medical records. Every patient, or former patient, is entitled to inspect, or to receive for a reasonable fee, a copy of his or her medical record upon request, within a reasonable timeframe.
· Confidentiality of records. Every patient, or former patient, is entitled to confidential treatment of his or her personal and medical records. A third party shall not be allowed to access to a patients personal and medical records without the prior authorization of the patient, except as required because of a transfer to another health care facility or as required by law or third party payment contract.
· Privacy. Every patient is entitled to privacy, to the extent feasible in treatment and in caring for his/her personal needs
· Refusal of treatment. Every patient is entitled to refuse treatment to the extent provided by law and to be informed of the consequences of that refusal. However, when a refusal of treatment prevents the physician from providing appropriate care according to ethical and professional standards, the physician may terminate the relationship with the patient.
· Explanation of bill. Every patient is entitled to receive and explanation of his or her bill, regardless of the source of payment.
· Personal safety. Every patient is entitled to expect reasonable safety as to Meadowbrook Urgent Care’s practices and environment.
· Transfer of care . No patient shall be transferred to another facility unless the patient has received a complete explanation of the need to transfer.
· HIV testing. No patient shall be tested for HIV infection without the patient’s consent, unless a health care professional or Meadowbrook Urgent Care employee sustains percutaneous, mucous membrane or open wound exposure to the patient’s blood or other bodily fluids. If such exposure has occurred and HIV test may be performed upon the patient, without the consent of the patient’s consent.
· Freedom from restraint and abuse. Every patient is entitled to be free from mental and physical abuse and from physical and chemical restraints, except those restraints authorized by the patient’s physician in the medical record for a specific and limited time, or as necessary by an emergency to protect the patient from injury to self or to other.
Patient Responsibilities
· Medical history. Each patient is responsible for providing honest and complete information about his or her current condition, past medical condition and treatment and/or any recent diagnosis.
· Lack of understanding. Each patient is responsible for making it known if he or she does not understand the description of his or her condition, or the description of the course of treatment proposed for that condition.
· Compliance. Each patient is responsible for following the treatment plan recommended by the physician.
· Charges for treatment. Regardless of the patient’s type of insurance, the patient is responsible for paying for, or ensuring payment for the medical treatment administered by Meadowbrook Urgent Care.
· Insurance coverage. It is the responsibility of each patient with health care insurance to know the benefits and exclusions of his or her health insurance plan.
· Updated demographics. It is the responsibility of each patient to notify Meadowbrook Urgent Care of any changes in address, phone number and insurance information in a timely manner.
